Good health depends on more than just seeing a doctor. Our health is also strongly impacted by where we live and work. Health Partners is a clinic-based program that works one-on-one with patients and their providers to help move people towards greater well-being.
Our team is here to ensure patients have the information they need to access community resources. Health Partners connects patients to a variety of social services such as:
- Housing assistance
- Job training
- Hot meals and food boxes
- Vision care
- Childcare
- Assistance with reducing medication costs
- Affordable heating in the winter
- Clothing
- Low cost and free cell phones
- Legal assistance
Desk hours: 9 am – 5 pm, Monday through Friday
