Health Partners is a program based on the principal that health is strongly impacted by an individuals’ social determinants of health, which when addressed, can move people towards greater well-being. A community-based resource desk, located between the FHA offices in front of the Flagstaff 4th Street East clinic, is staffed by NAU pre-med & public health student interns. These interns work one-on-one with North Country Healthcare patients to connect them with community resources such as:
