May 2024 AZ Community Grand Rounds: SDOH Connections with CommunityCares This session will include an overview of the CommunityCares closed-loop referral platform and how it streamlines information sharing and strengthens a social determinants of health (SDOH) network throughout Arizona. Our panel will include perspectives from a local provider, Phoenix Children’s Medical Group, and a community-based organization, A New Leaf, who will share their work supporting SDOH and their experience with referral coordination through CommunityCares.
